For many estate planning attorneys, an Advance Health Care Directive (AHCD) is just an add-on trust estate document.  For others, the AHCD has very detailed language, language that sounds good, but which can be disastrous at the time of need.  Choosing not to prolong life may very well mean something different if the issue is advanced dementia as compared to a heart attack or cancer.  Advanced Care Planning, where the AHCD is but one tool, should be considered.  We will discuss meaningful ways to approach the various issues that may arise from Advance Care Planning and, hopefully, share real world examples of what has worked and what does not work.
